Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.  We will ask for your ID and your insurance card while you fill out the new patient paperwork including your past medical history, past surgical history, your social history, allergies, and a list of medications.  Typically in your first visit we are able to review your complaints; we like to limit each visit to the top two complaints at the time.  We are equipped with an electronic medical record (EMR) and, if necessary, Digital X Ray that we review with your in the office. We are also able to prescribe medications electronically for efficient ordering of your prescriptions.  Also, take some time to review our staff page and familiarize yourself with the doctors.   We look forward to your first visit.
